Business information

  • Legal business name and address
  • Type of business entity and the year it was established
  • Business Tax ID number
  • Number of employees

Most recent fiscal year financial statement, including balance sheet and income statement with the following information:

  • Gross Revenues, Net Income, Depreciation and Interest Expense
  • Current and Total Assets
  • Current and Total Liabilities

Most of the information listed above would be contained in your most recent business tax return or financial statements.

Business owner information

  • Name and home address
  • Social Security Number
  • Title and ownership percentage

A government-issued photo ID (such as a driver’s license, U.S. passport, or state ID)
