Official Rules for Lake City Bank’s “Take the Spring Break You Really Want” $10,000 Debit Card Sweepstakes
Please review Lake City Bank’s “Take the Spring Break You Really Want” $10,000 Debit Card Sweepstakes (“Sweepstakes”) Official Rules (“Official Rules”) before entering the Sweepstakes. Participation constitutes entrant’s full and unconditional agreement to these Official Rules. Lake City Bank’s decisions are final and binding in all matters related to the Sweepstakes. Winning the Sweepstakes is contingent upon fulfilling all requirements set forth herein.
A.) Eligibility
This Sweepstakes is open only to Indiana residents who are at least eighteen (18) years old at the time of entry. Employees, officers, and directors of Lake City Bank, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members (spouse, parents, siblings, and children) and household members are not eligible. The Sweepstakes is subject to laws of the State of Indiana and is void where prohibited. No purchase or payment is necessary to enter the Sweepstakes. A purchase or payment will not increase an entry’s chances of winning.
B.) Sweepstakes Period
The Sweepstakes entry period begins at 12:00 a.m. ET on February 1, 2026, and ends at 11:59 p.m. ET on February 28, 2026 (“Sweepstakes Period”).
C.) How to Enter
Automatic Entry: One (1) entry earned for each eligible Lake City Bank Visa® debit card purchase transaction that posts to your Lake City Bank deposit account during the Sweepstakes Period. Transactions made using Lake City Bank business debit cards, Health Savings Account debit cards, Home Equity Line of Credit access cards, and ATM cards are not eligible for the Sweepstakes and will not count as entries. Eligible purchase transactions must be considered “purchase transactions”, which are generally point-of-sale purchase and purchase with cash back transactions. Account funding transactions, including, without limitation, person-to-person money transfers (e.g. Zelle® transfers), account-to-account money transfers, certain small business disbursements to entities offering payment services to small business (e.g. payroll, invoice payments), pre-funding of a staged digital wallet, stored value wallet funding, prepaid or reload of stored value cards, and acquisitions of stocks, shares, foreign currency, and cryptocurrency are expressly not eligible for entry. ATM withdrawals are not eligible for entry. Eligibility determinations shall be made by Lake City Bank at its sole discretion.
Alternative Method of Entry (“AMOE”): You may enter the Sweepstakes without making a purchase or payment either by completing the in-branch AMOE form in person at any Lake City Bank office location during normal business hours or following the AMOE mail-in entry instructions listed below.
The in-branch AMOE form requires you to hand print your full name, address, phone number, and email address. Lake City Bank reserves the right to use information provided on the in-branch AMOE form for marketing and promotional purposes of its products and services, and is subject to Lake City Bank’s Privacy Policy, which is available at https://secure.lakecitybank.com/pickup/privacy-policy.pdf.
Alternatively, you may mail a piece of paper with your full name, address, telephone number and email address, in a self-addressed and postmarked envelope to Lake City Bank, ATTN: Debit Card Sweepstakes, P.O. Box 1387, Warsaw, IN 46581. The envelope must be postmarked between February 1-28, 2026 to be eligible for entry. Lake City Bank reserves the right to use information provided on mailed-in AMOE entries for marketing and promotional purposes of its products and services, and is subject to Lake City Bank’s Privacy Policy, which is available at https://secure.lakecitybank.com/pickup/privacy-policy.pdf.
Limit one (1) AMOE entry per person regardless of the AMOE method. You do not have to be a Lake City Bank customer to submit an AMOE entry. Duplicate entries regardless of the AMOE method will be void. Incomplete or illegible AMOE entries will not be accepted.
D.) Odds of Winning
Odds of winning depend on the total number of eligible entries received.
E.) Prize
One (1) Prize of $10,000 (“Prize”) will be awarded via check made payable to one (1) Winner among eligible entries (“Winner”). Prize is non-transferable and no substitution is allowed except at Lake City Bank’s discretion. Winner is responsible for all applicable taxes, must provide an IRS Form W-9 to Lake City Bank, and will receive IRS Form 1099 for Prize value. By accepting this Prize, the Winner is consenting to receive an IRS Form 1099 reflecting the actual value of the Prize received.
Prize will be fulfilled within approximately thirty (30) days after Winner confirms acceptance of the Prize and provides all requested documents and information to Lake City Bank within required time periods set forth herein. Receiving the Prize is contingent upon compliance with these Official Rules.
F.) Winner Selection and Notification
Winner will be selected in a random drawing on or about March 10, 2026, from all eligible entries. A maximum of three (3) attempts via phone and/or email will be made to contact the potential Winner within five (5) business days (“Contact Period”) of the drawing. The Winner must acknowledge acceptance of the Prize within the Contact Period via phone, email, or in-person verbal acknowledgement. The Winner must complete, sign, and return IRS Form W-9 (Request for Taxpayer Identification Number and Certification). The Winner must sign and return the Declaration of Compliance, Liability Waiver, and Publicity Release within seven (7) days (“Claim Period”) of the date of Prize acceptance. If a potential Winner cannot be contacted, fails to confirm acceptance of the Prize within the Contact Period, or fails to provide the required forms or information within the Claim Period, the potential Winner forfeits the Prize.
Lake City Bank reserves the right to complete its standard customer identification and verification process before issuing the Prize.
If a potential Winner is disqualified or forfeits the Prize for any reason, Lake City Bank will award the Prize to an alternate Winner by random drawing from among all remaining eligible entries from the Sweepstakes Period. Only three (3) alternate drawings will be held, after which the Prize will remain un-awarded.
G.) Publicity of Winner
By accepting the Prize, Winner grants Lake City Bank permission to use their name, image, likeness, and entry details for promotional purposes without additional compensation. To claim the Prize, the winner must comply with Lake City Bank’s requests to pose for a photo opportunity and consent to Lake City Bank sharing photographs and information about the Winner on its social media profiles, website, and other communication channels at Lake City Bank’s sole discretion.
H.) General Conditions
Lake City Bank reserves the right to cancel, suspend and/or modify the Sweepstakes, if any fraud, technical failures, human error, or any other factor in its discretion impairs the integrity or proper functioning of the Sweepstakes, or any event or cause beyond Lake City Bank’s control (e.g. events such as natural calamities, national emergencies, wide spread illnesses, declarations of war, acts of God, acts of terrorism) interferes with any aspect of the Sweepstakes, including but not limited to fulfillment of the Prize, as determined by Lake City Bank in its sole discretion. In such event, Lake City Bank, in its sole discretion, may elect to hold a random drawing from among all eligible entries received up to the date of discontinuance and may modify the Prize offered herein. Lake City Bank reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or to be acting in violation of the Official Rules of this or any other promotion or in an unsportsmanlike or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Sweepstakes may be a violation of criminal and civil law, and should such an attempt be made, Lake City Bank reserves the right to seek damages from any such person to the fullest extent permitted by law. Lake City Bank’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
I.) Disputes and Liability
The Sweepstakes Official Rules shall be governed and construed by the laws of the State of Indiana, without regard to its conflict of laws provisions. You hereby consent and submit to the personal and exclusive jurisdiction of the courts of the State of Indiana located in Indianapolis, county of Marion, in any action or dispute related to the Sweepstakes Official Rules. By participating in the Sweepstakes, you agree to release and indemnify Lake City Bank and its employees, affiliates, and agents from any liability, losses, damages, and claims of any kind in connection with the Sweepstakes.
